Act Now Foundation offers free Alzheimer’s and related dementia services to individuals living with memory loss and their caregivers. Whether you're facing a new diagnosis or navigating long-term care, we’re here to help you plan ahead, find support, and feel empowered every step of the way.

All programs are offered at no cost and funded in part by the Hudson County Office on Aging, the Hudson County Office of Inclusion and Accessibility, the Bergen County Division of Senior Services, the Essex County Division of Senior Services, and Community Development and Services Block Grants.

Cognitive Assessments

Understand memory loss early and take action.

Our free memory screenings help identify signs of Alzheimer’s or related dementias so you can make timely medical, legal, and care decisions. Screenings are available in-office, in-home, or as part of community events.
